Patient Forms
If you are a new patient to our office, please visit our patient forms page to download our new patient bundle with forms that will need to be filled out when you arrive at our office. Printing them, filling them out and bringing them with you will allow us to attend to your medical needs more quickly than completing them on your arrival. Payment Options
For your convenience, we accept cash, personal checks, debit cards, and most major credit cards. Payment is expected at the time services are performed. Feel free to contact our office if your treatment plan requires more comprehensive dental work and you want to discuss financial arrangements.
Dental Insurance
At Adair Dentistry, we strive to provide the highest quality of care for every patient, basing our diagnoses on your specific oral healthcare needs. We are happy to accept assignment of benefits from most dental insurance companies. Our knowledgeable staff will help you determine and understand your insurance coverage. There may be times when your insurance carrier does not provide coverage for certain procedures even though they are necessary for your oral health. In these instances, we will be happy to work out a payment arrangement with you.
